LIST OF DOCUMENTS TO OBTAIN AN OFFICIAL LETTER OF THE MINISTRY OF HEALTH (CERTIFICATE OF GOOD STANDING)

According to Item 7.1 of the list of administrative procedures performed by state bodies and other organizations on the applications of citizens, approved by the Decree of the President of the Republic of Belarus dated 26.04.2010 under No. 200 “On administrative procedures undertaken by government agencies and other organizations of citizens”, the Ministry of Health of the Republic of Belarus does not issue a Certificate of Good Standing.

List of documents to obtain an official letter of the Ministry of Health about medical/pharmaceutical education of the persons and their ability to exercise medical/pharmaceutical practice within the territory of the Republic of Belarus to be presented to official bodies of the foreign states.

To obtain an official letter of the Ministry of Health of the Republic of Belarus about medical, pharmaceutical education of the persons and their ability to exercise medical, pharmaceutical practice within the territory of the Republic of Belarus to be presented to official bodies of the foreign states the applicant shall submit the following documents to the Ministry of Health:

- a personal application with a request to receive a corresponding letter of the Ministry of Health;

- a reference letter from last place of employment (original document), in case the applicant has had several jobs over the last five years – then reference letters must be from each place of employment; (the reference letter must be arranged, signed by the head of the organization and stamped by the employer according to Item 17 of the Instructions on the procedure of formation, maintenance and storage of personal files of employees, approved by the Resolution of the Committee on archives and records management of the Council of Ministers of the Republic of Belarus dated 26.03.2004 under No. 2)* (see the footnote);

- a Certificate confirming no criminal/ conviction record issued by the municipal (regional) Department of internal affairs at the place of residence of the applicant (original document);

- an archival certificate from the educational institution where the applicant received a higher (secondary special) medical/pharmaceutical education (original document);

- a copy of identity document (passport or Republic of Belarus residence permit) certified in accordance with applicable legislation;

- a copy of diploma of higher (vocational) medical/pharmaceutical education certified in accordance with applicable legislation;

- a copy of certificate of internship (for physicians or pharmacists-specialists) certified in accordance with applicable legislation;

- a copy of the work record book certified in accordance with applicable legislation;

- copies of the documents confirming assigned qualification grades, further education, retraining, training in clinical residency and other documents related to professional activities certified in accordance with applicable legislation (when available);

- a copy of a marriage certificate, certificate of change of name and etc. certified in accordance with applicable legislation (if the applicant has changed his/her surname, name, patronymic).
The applicant or his/her official representative shall personally submit the documents to obtain an official letter of the Ministry of Health during reception days of the Head of the Board or the Deputy Head of the Board. The documents may as well be sent via post to the address: 39 Myasnikova St., Minsk, 220048.

* Reference letter shall be issued on a standard A4 sheet of paper or a common form in any manner.

It shall bear the following information:

1) name of the document type (reference letter);

2) name, middle name, last name of the person being characterized;

3) text bearing the following information:

 - year of birth;
 - education;
 - position occupied;
 - structural subdivision;
 - employment history in this organization and in this position;
 - business proficiency and moral qualities;
 - estimations of professional experience;
 - improvement of professional skills;
 - availability of incentives, state awards;
 - purpose of the reference letter .

External reference letter (aimed to be submitted to another organization, including for those persons leaving the Republic of Belarus for the purpose of medical activities) shall be signed by the head of the institution and have a date and seal of the employer.
